Arggh! That question has been bugging me for YEARS!!!! A Memory of Light is on its way!
It was revealed in Towers of Midnight. I don't think it was handled very well though; it was implied as an aside and then stated straight out in the glossary.
well was that Grendal? I somehow missed it .... (I was listening to audiobook, so no glossary)...

*POTENTIAL SPOILER WARNING* - answer to question below:
Yes. At one point Moridin criticises Graendal for bringing about Aran'gar's death, claiming that she is "making a habit" of killing other Forsaken. A habit implies it's happened at least once before and the only potential candidate is Asmodean. At the end of the book, her entry in the glossary states: "Graendal: A ruthless killer, she was responsible for the deaths of Aran'gar and Asmodean and for the destruction of Mesaana."
responsible... could be, but did she do it?
The "habbit" involves Aran'gar and Mesaana.... but Asmodean... it is still unclear... I don't think either Jordan or Sanderson are so simple. I bet it's gonna be something we did not expect (or at least I hope so).......................
The quote about the deaths of other Forsaken becoming a habit occurs before Mesaana's downfall in the book, so it doesn't refer to her; it's definitely Asmodean. "She was responsible for" does leave a bit of leeway, but it's most likely been phrased that way because of Aran'gar, whom Graendal didn't directly kill, rather than Asmodean.
Sanderson also claimed before ToM was published that it would reveal Asmodean's killer, by the way, and I believe has confirmed that since the publication.
And all this time I thought it was Professor Snape.

Graendal doesn't surprise me... she's extremely capable and it had to have been someone that Asmodean knew since he recognized his killer before he/she killed him.
On the other hand.... nothing that smurfs do would surprise me either!
